Agreement to Terms

By downloading, installing, or using the “My Health” mobile application (“App”) provided by Data Integrities, Inc. (“Company,” “we,” “our,” or “us”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree to these Terms, do not use the App.

Important Healthcare Disclaimer

Development Status: The My Health app is currently in development. All health information displayed is simulated for demonstration purposes only. This app does not currently connect to real healthcare systems or process actual patient data.

Not Medical Advice: This app is not intended to provide medical advice, diagnosis, or treatment. Always seek the advice of qualified healthcare providers for medical conditions.

Future Functionality: Upon commercial release, this app will connect to Epic health systems to provide enhanced healthcare management features.

Description of Service

  • User interface demonstration for healthcare management
  • Simulated health data displays

User Responsibilities

Account Security

  • Maintain the confidentiality of your login credentials
  • Notify us immediately of any unauthorized access
  • Use strong, unique passwords for your account
  • Log out from shared devices

Appropriate Use

You agree to use the App only for its intended healthcare management purposes and not to:

  • Attempt to access unauthorized data or systems
  • Interfere with the App’s operation or security features
  • Use the App for any illegal or unauthorized purposes
  • Share your account credentials with others
  • Reverse engineer or attempt to extract source code

Healthcare Responsibilities

When connecting to real healthcare systems (future):

  • Ensure you have proper authorization to access health information
  • Use the App in compliance with your healthcare provider’s policies
  • Report any suspected privacy or security incidents immediately

Privacy and Data Protection

Your privacy is important to us. Please review our Privacy Policy at dataintegrities.com/privacy for information about how we collect, use, and protect your information.

HIPAA Compliance: Upon production release, all health data handling will be HIPAA compliant with appropriate Business Associate Agreements in place.

Intellectual Property

Our Rights

The App, including all content, features, and functionality, is owned by Data Integrities, Inc. and is protected by intellectual property laws. This includes:

  • Software code and architecture
  • User interface designs
  • Logos and branding materials
  • Documentation and help content

Your Rights

You are granted a limited, non-exclusive, non-transferable license to use the App for its intended purpose. This license does not include the right to:

  • Copy, modify, or distribute the App
  • Create derivative works
  • Use our trademarks or branding
  • Access or use the App after termination

Technology Disclaimer

THE APP IS PROVIDED “AS IS” WITHOUT WARRANTIES OF ANY KIND. WE DO NOT GUARANTEE:

  • Uninterrupted or error-free operation
  • Compatibility with all devices or systems
  • Accuracy of all displayed information (especially during development)
  • Availability of third-party healthcare systems

Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, DATA INTEGRITIES, INC. SHALL NOT BE LIABLE FOR:

  • Any indirect, incidental, special, or consequential damages
  • Loss of profits, data, or business opportunities
  • Medical decisions made based on App information
  • Third-party system failures or data breaches
  • Damages exceeding the amount paid for the App (if any)

Healthcare Provider Relationships

No Provider Relationship

Use of this App does not create a patient-provider relationship between you and Data Integrities, Inc. We are a technology company, not a healthcare provider.

Epic Integration

When available, Epic integration will be subject to:

  • Your healthcare provider’s Epic system policies
  • Appropriate user authentication and authorization
  • Healthcare provider approval for app connections
  • Compliance with all applicable healthcare regulations

Termination

Your Right to Terminate

You may stop using the App at any time by uninstalling it from your device.

Our Right to Terminate

We may terminate your access if you:

  • Violate these Terms
  • Use the App inappropriately or unsafely
  • Engage in fraudulent or illegal activities
  • Pose a security risk to our systems

Effect of Termination

Upon termination:

  • Your right to use the App ends immediately
  • We may delete your account and associated data (subject to legal requirements)
  • Provisions regarding intellectual property and limitations of liability survive

Updates and Changes

App Updates

We may update the App to:

  • Add new features and functionality
  • Improve security and performance
  • Fix bugs and technical issues
  • Ensure compatibility with updated systems

Terms Updates

We may modify these Terms with notice to users through:

  • In-app notifications
  • Email communications
  • Website posting at dataintegrities.com
  • App store update descriptions

Continued use after notice constitutes acceptance of updated Terms.

Geographic Limitations

The App is developed and operated in the United States. Use outside the US may be subject to local laws and regulations. Healthcare integrations will initially be limited to US-based Epic systems.

Dispute Resolution

Governing Law

These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.

Dispute Process

Before filing any legal action, you agree to:

  1. Contact us directly to attempt resolution
  2. Participate in good faith discussions for 30 days
  3. Consider mediation for remaining disputes

Jurisdiction

Any legal proceedings will be conducted in the state or federal courts of Dallas County, Texas.

Severability

If any provision of these Terms is found to be unenforceable, the remaining provisions will remain in full force and effect.

Entire Agreement

These Terms, together with our Privacy Policy, constitute the entire agreement between you and Data Integrities, Inc. regarding the App.


By using the My Health app, you acknowledge that you have read, understood, and agree to be bound by these Terms of Service.
